Telemedicine Services for Better Health

  • CREATE has been bringing good health to the doorsteps of the most marginalized through Tele Medicine initiatives, often run in collaboration with government systems, to enhance sustainability. It has running such interventions with expertise in different parts of India, such as Tamil Nadu, Maharashtra, Madhya Pradesh etc.

  • Through such interventions CREATE is providing access to specialized and low cost medical services to the marginalized section of community who are otherwise unable to access or afford such services. Thus CREATE is ensuring better health and wellness condition for the members of the marginalized communities. Often these interventions have community level mobilizers and health cadres, trained by CREATE and the intervention is

CREATE has been bringing good health to the doorsteps of the most marginalized through Tele Medicine initiatives, often run in collaboration with government systems, to enhance sustainability. It has run such interventions with expertise in different parts of India, such as Tamil Nadu, Maharashtra, Madhya Pradesh Uttar Pradesh and Uttrakhand etc.

Through such interventions CREATE  provided access to specialized and low cost medical services to the marginalized section of community who are otherwise unable to access or afford such services. Thus CREATE is ensuring better health and wellness condition for the members of the marginalized communities. Often these interventions have community level mobilizers and health cadres, trained by CREATE and the intervention is gradually linked to government medical facilities to ensure sustainability.

  • 7 districts, 13 blocks and  villages have benefitted from CREATE’s tele-mediine interventions. CREATE has impacted 50,000 lives through this intervention. All being  members from the marginalized sections of the community .The services rendered were of medical specialists, such as dermatologists, orthopedics, pediatrics, gynecologists and ENTs. It may not have been possible for them to access such specialist care otherwise. Collectivization and selection of trained members from the community to mobilize the community members were part of core approaches.

The no-cost medical tests and medical check-ups were lifesaving for members of the community, who do not have such medical facilities in the proximity and who find the cost of such facilities, where they are available, prohibitive. As a result of the intervention, better health and wellness condition of the marginalized community has been noticed.

Core services of our telemedicine interventions include:

Contribution towards Replacement TFR

  • CREATE did an intervention on the promotion of Female Contraceptive (An Initiative by ABT Associates: In Association and support with BMGF/HP). This initiative was  run in entire state of Uttar Pradesh and 19 districts of Bihar.. The intervention aimed at reducing TFR rates and also issues related to frequent child births and also STDs through the use of the female contraceptive.

AIDS Control Targeted Initiative

  • This is an initiative since 2006 on  Targeted Health Intervention on HIV/AIDS in Lucknow district. The core beneficiaries of this intervention have been the high risk groups/HRGs within the communities and the multi-pronged initiative is helping to reduce their risks from HIV/AIDS through both prevention and medical care.

Better health consciousness and awareness among the target HRGs and better protection measures adopted by them has ensured that HIV/AIDS transmission has come down to a negligible level in the area. More than 4750 high risk group members (1400 FSWs) benefitted as a consequence of this intervention.

Nand Ghar Initiative

  • CREATE implemented a systematic and transformative program with the Anganwadi Centres named Nandghars and worked to upgrade the ICDS program in 425 Anganwadis in Uttar Pradesh. CREATE identified the resource and functional gaps in the target Nand Ghars and closed these gaps ensuring better Nand Ghar performance in health, nutrition, education and skill building. It also monitored to ensure sustained quality at these Nand Ghars.

Through CREATE’s efforts the Anganwadis became a modern nucleus of development. Today these anganwadies, called Nand Ghars, have access to clean drinking water, toilets, garbage disposal facility etc. CREATE’s efforts have also ensured availability and safety of assets such as weighing machines etc. Better performance in distribution of supplementary nutrition, enrolment and regularity of children at pre-school non-formal schooling, immunization, referral check-ups and skill building etc. was gradually achieved through the sustained effort of the CREATE team.

A comparison with baseline data confirms the changes CREATE has brought about. In the Nand Ghars CREATE nurtured THR is distributed regularly and all of them have clean drinking water. WASH practices, which were poor before, are today regularly practiced by the children.

Overall, 12750 infants and children and 5100 women have benefitted as a result of this intervention.

Livelihood Interventions

High Impact CSR Project with innovative Technology for getting huge environment benefits while doubling Farmers income – An IITian Startup:
  • Often Indian farmers are not able to access new technologies in an efficient manner. As a result, their productivity is relatively low. Other challenges involved with agriculture in India are that food production is the second largest cause of GHG emissions such as CO2 emissions.

This is exactly where CREATE made a positive difference by introducing Urea Deep Placement/UDP technology. The focus areas of the intervention are: facilitating access to new technologies such as FDP/UDP among farmers, especially the small and marginal farmers; creating better agricultural practices that reduce fertilizer and urea usage and increase agricultural yield and returns; improving returns from livelihood of marginalized communities and farmers; reducing greenhouse gases.

CREATE is carrying out its UDP/FDP initiatives in 03 districts , 06 blocks and 57 villages in the states of Haryana and Uttar Pradesh. CREATE selected farmers and trained them at the designated project location. Awareness camps and training sessions were organized to educate farmers about UDP Technology, followed by on-field demonstrations.

57 UDP applicators and 15 briquetting machines have been successfully delivered to the 57 villages under the program.

  • Stakeholder collectivization and involvement has been a key to these initiatives. Village committees have been formed by CREATE in each of these villages. These committees have been strengthened and hand held to become the custodians of the new technology equipment, after withdrawal of the program. This step aims to empower local communities to take a more active role in the UDP project and enhance its sustainability.

As a result of CREATE’s efforts, urea usage has decreased by 30 percent in the UDP-treated fields compared to control fields. An assessment exercise showcased a promising increase of 15-20 percent in crop yield.

Sustainable Livelihood Initiative through farmers’ collectives

  • CREATE has played a strong role in enhancing the livelihood of farmers. One such initiative is the SFURTI initiative, which was initiated in the two villages of Baraulia and Hariharpur in the Amethi district of Uttar Pradesh.

The focus areas of the intervention are: Educating and building the capacities of the local farmers to enable them to grow high return crops and learn about new farming techniques; Collectivizing farmers to benefit from scale and joint mechanisms of production, processing and forward linkages to leverage better profits from farming; also ensuring sustainability through handing over such a crucial unit to farmers’ collectives.

Sowing of medicinal plants such as Mint, Coriander, Lemon Grass, Turmeric, Vetiver, Basil, Menthe and Drumstick were initiated in a planned and systematic manner. A standard package of practice was established for each crop with the help of senior scientists and experienced practitioners.

As a result of CREATE’s efforts, today approximately 654 farmers are linked with this initiative and benefitting from it either directly or indirectly. The Common Facility Centres are a platform for the farmers to directly sell their crops to the buyers, thus increasing profitability for the farmers. These Centres are also providing opportunity for employment to 50 farmers. This is now working as a sustainable model by creating the ‘Pavni’ brand of spices, which is selling in these localities and generating income for the stakeholders

Education Interventions
_______________________________________________________________________________

Introducing Digital Inclusion

  • Emphasis and need on digital inclusion is constantly increasing, particularly so with the integration of AI in modern life. Integrating technology in education is thus the need of the hour. Here CREATE is reducing the digital divide and providing digital up-gradation in schools for children from the marginalized communities, thus enhancing their 21st century skills and future readiness.

  • Here CREATE has worked with both teachers and students to integrate them with the digitalized education systems in schools. It has introduced a digital platform to enhance teaching learning processes in the target school. This intervention is being carried out in a government school in Lucknow, Uttar Pradesh.

  • As a result of CREATE’s efforts through digital applications, the students now have improved understanding of digital technology and digitally access vast knowledge related to their curriculum. They can record classes and upload it on the application, so that the overall process is more accessible and interesting for the children. This app has also enabled the teachers to monitor the students’ progress better.

Strengthening Pre-School Education

  • The Nand Ghar intervention for which CREATE was partnered in the districts of Amethi, Rae Bareilly and Hapur, is a unique, multi-pronged intervention. The Nand Ghars are essentially Anganwadi Centres with which CREATE has intervened to enhance their infrastructure, functionality and governance mechanisms.

  •

Here CREATE worked closely with parents of children in the age group of 3-6 years and motivated the parents and community to send the children to the Nand Ghars. To retain the children, CREATE team has made the school environment more appealing and teaching learning process more child-friendly. Child friendly teaching devices, practices and monthly events have played crucial roles and LED display installed by CREATE in these Nand Ghars also created a lot of excitement among the children.

  •

Through CREATE’s efforts the parents are also now motivated and attending PTM/Parents’ Teachers’ Meeting at the Nand Ghars. This involvement of parents in their children’s pre-school and Nand Ghar governance is unforeseen in the region. As a result of CREATE’s efforts the attendance of children has also gone up from approximately 35 percent to approximately 80 percent.

Women Empowerment
_______________________________________________________________________________

Introducing Digital Inclusion

  • UPSRLM and DA have played a strong role in developing SHG linked village, block and cluster level federation of women in rural setup. Now CREATE is playing a crucial role towards capacity building of women and developing their entrepreneurial skills and mindsets. Eventually this will foster their leadership skills, voice and choice, power of independent thinking, entrepreneurial skills and empower them overall and also financially.

  •

CREATE has already developed three training modules for the cluster leaders and has developed a team to facilitate their capacity building in their entrepreneurship journey. These training modules cover self-transformation, skilling and collaboration as entrepreneurs.

  • CREATE’s role will be pivotal in facilitating the Three-Dimensional Empowerment Theory:

Capacity Building Role
_______________________________________________________________________________

  • Capacity building of NGOs and youth, both through face-to-face workshops and online, is gradually emerging as CREATE’s forte.

  •

CREATE is part of the Joint Staff Development Program Partners/JSDP Network Partners and is powered by Konrad Adenaur Stiftung, Germany India Office. This network consists of approximately a dozen NGOs across India. Here CREATE has been playing a crucial leadership and anchoring role over the years. Various topics are covered in their annual workshops and CREATE continues to play a pivotal role in building the capacity of NGOs on various topics.

  •  Over the years CREATE has facilitated the Annual Conference of JSDP Network Partners. The Annual Conference for 2023-24 was held in Mysore, Karnataka The topic for this conference was ‘The role of Civil Societies in Promotion of SDGs on Water, Energy and Climate Action’.

  •

CREATE is also an integral part of workshops anchored by Vishwa Yuvak Kendra and facilitated by CREATE. NGO representatives from different parts of Uttar Pradesh participate in these workshops. These workshops have covered different topics over the years, such as ‘Capacity Building of NGOs on Corporate Social Responsibility: Engaging NGOs for Impactful Change’.

  •  Besides this, CREATE is also part of the capacity building initiative by IMEDF and has conducted online workshop ‘E-commerce as a Growth Driver’.

Outcomes & Impact

  • CREATE has played a substantial role in enterprise creation. The organisation has covered 65 districts and organized over 200 entrepreneurship development programs of 15 days, 30 days and 45 days. These have a reported success rate of over 40%. The entrepreneurship development initiatives were supported by NABARD, SIDBI and Directorate of industries.
  • We also played a key role in Polio eradication initiatives in 2 blocks of Gonda district.
  • We ensured better income for around 1000 rickshaw pullers through a micro finance initiative in Agra. This has  enabled them to buy their own rickshaws/trolley/push kart.
  • CREATE has facilitated and strengthened the WASH program, with special emphasis on the issues regarding girl child in as many as 60 schools.
  • Have been catalytic to formation of 800 SHGs, through whom 10000 women are linked to micro finance. As many as 9000 of these women are generating income through loans taken from their groups and bank linkage of the groups.
  • CREATE has been able to impact reproductive health choices of 3 lakh women.
  • The organisation has improved HIV/AIDS health seeking behaviour among 2000 people and reduced opportunistic infections by 15 percent.
